This Medical Card is to be completed and printed out by each California Legacy Run participant. The card will be placed in a lanyard provided at check-in for use in the event of a medical emergency. (Click on the link on the left)
This form is required to allow a MINOR participant to compete, officiate, observe, work for, or participate in any way in the 2024 California Legacy Run event and/or activities.

“This form is only for those participants who don’t want to register online. Please present this completed form with registration fee at check-in. Be sure to have all identification, vehicle registration and insurance documents with you.”
